PROSTHETIC ARTHROPLASTY OF THE CARPAL SCAPHOID

Thedifficulties of diagnosing the vascularization of the fragments of an unconsolidated sca-phoid persist in spite of the use of magnetic resonance imaging. Likewise, the treatment of pseudoarthroses in which the proximal pole is so affected structurally that a conventional or vascularized bone graft must be used remains unresolved. A short historical review is made of different designs and materials used intotal or partial scaphoid prosthesis. Our own experience with partial silastic prosthesis is touched upon. Unfortunately, the use of such prostheses in young people with heavy functional demands does not yield reliable long-term results. Therefore, the search continues for a prosthetic material better able to meet such requirements.

RESUMEN: Las dificultades para diagnosticar elgrado de vascularización de los fragmentos de un escafoides no consolidado permanecen pese al empleo de la resonancia magnética. De igual manera sigue sin resolverse el tratamiento de aquellas pseudoartrosis en las que el polo proximal está afectado estructuralmente en tal grado que resulta inútil la utilización de un injerto óseo, ya sea convencional o vascularizado. Exponemos en el presenteartículo una breve revisión histórica de los diferentes diseños y materiales de prótesis totales o parciales de escafoides utilizadas, añadiendo los datos básicos de nuestra experiencia con la prótesis parcial con vástago de silastic. Lamentablemente hasta el momento ninguna de las prótesis utilizadas ha proporcionado un resultado funcional fiable a largo plazo si se utilizan en pacientes jóvenes conaltas exigencias funcionales. Se hace imprescindible seguir investigando en busca de un material protésico más idóneo para soportar dichas exigencias.
