BLOOD-BRAIN BARRIER: DEVELOPMENT OF A STRUCTURE WHICH SUPPORTS
THE FUNCTIONAL HETEROGENEITY OF THE CENTRAL NERVOUS SYSTEM
Summary. Aims. To analyze the functionalreasons justifying the existence of the blood-brain barrier with an emphasis on its
fundamental role supporting neuroglial coupling. Development. We review in an integrated manner the contributions of
different research areas in physiology and metabolism of the central nervous system which allow to understand the functional
need for the existence of the blood-brain barrier. In particular, wedescribe the physiological basis of the metabolic-functional
coupling and the metabolic interactions between neurons and glial cells, two properties directly derived from the presence of
the blood-brain barrier. Likewise the blood-brain barrier is presented as an important determinant of the heterogeneous
activation of cerebral tissue as detected by neuroimaging technologies as positron emissiontomography and functional
magnetic resonance imaging. Conclusions. The main function of the blood-brain barrier is to maintain a stable composition of
the extracellular milieu in nervous tissue. This allows the changes in ionic composition and neurotransmitter concentration in
the extracellular milieu, to reflect indirectly the generation of action potentials and the state of neurotransmission ofneuronal
circuits. Glial cells induce the development of the blood-brain barrier and are the main sensors of neuronal function, due to
their important take up capacity for extracellular potassium and neurotransmitters. Glial homeostasis of the extracellular
milieu is circuit-specific, limiting the functional-metabolic coupling to discrete regions of the brain and generating the classicalpattern of heterogeneous activity in the different modules of the nervous tissue. [REV NEUROL 2004; 38: 565-81]

INTRODUCCIÓN
El sistema nervioso central (SNC) requiere para su funcionamiento de un aporte continuo de sustratos energéticos –fundamentalmente oxígenoy glucosa–, suministrados a través de
una riquísima red de capilares sanguíneos, probablemente la
más compleja del organismo. Sin embargo, y aparentemente de
forma paradójica, el medio interno del SNC se encuentra aislado con respecto a la circulación sanguínea por una estructura
también única en el organismo: la denominada barrera hematoencefálica (BHE) [1-4]. Esta barrera se nos hacepatente a los
especialistas clínicos cuando queremos tratar una patología
que afecta al SNC y, tras administrar un tratamiento farmacológico por vía intravascular (IV), nos encontramos con una ausencia de efecto terapéutico, ya que el fármaco no puede traspasar dicha barrera [5]. Este es el caso, por ejemplo, de muchos
antibióticos que, aunque sean los idóneos en la lucha contra el
germen causal deuna meningitis, no pueden escogerse por su
imposibilidad de paso a través de la barrera; o el de la mayoría
de los citostáticos, de nula utilidad en la lucha contra los tumores primarios del SNC por la misma razón [6]. Por otro lado,
los clínicos también conocemos los efectos de una disfunción
de la BHE, tal y como se manifiestan en el edema cerebral asociado a la mayoría de los tumoresmalignos del SNC, particu-

larmente en las metástasis. Estas lesiones inducen, a través de
factores angiogénicos, la formación rápida de nuevos vasos,
que son necesarios para sostener su crecimiento continuo. Sin
embargo, estos neovasos carecen de BHE y generan una trasudación de agua al tejido nervioso, o edema cerebral de origen
vasogénico, cuya consecuencia...
