Funciones ejecutivas: necesidad de una integración conceptual

EXECUTIVE FUNCTIONS: THE NEED FOR THE INTEGRATION OF CONCEPTS Summary. Introduction. The new cognitive neuropsychology approachs have aroused an increasing interest in understanding the higher cognitive processes as well as the neuralsubstrates linked to them. Particularly, the executive functions, reckoned to be essential to control the information processing and to co-ordinate behaviour, have received preferential treatment from specialised literature on the subject. Development. From obsessive-compulsive disorder to schizophrenia, from Parkinson’s disease to multiple sclerosis, there are many reports that show theaffectation of these functions in all these morbid processes. On the other hand, the part that the prefrontal cortex plays in human behaviour in general, and in executive functions in particular, constitutes one of the most important fields of research of neurosciences nowadays. Thus, this cortical area appears closely linked to the executive processes, affecting different respects of the cognitivefunctions. Working memory, supervisory attentional system, somatic marker, information processing, behaviour planning, social judgement, are processes which have been related to the prefrontal cortex activity as a structure, and to the executive processes as a function. Conclusions. The aim of this article is to revise the concept of executive functions, and give rise reflections about the usefulness ofthe aforementioned concept and its practical applicability. It is essential that we understand the difference between structure and function, cognition and emotion, brain activity and behaviour, category and dimension, and between mind and brain, to achieve a more comprehensive approach to this concept of ‘executive functions’ we all use, and many times find difficult to define and to understand.[REV NEUROL 2002; 34: 673-85] Key words. INTRODUCCIÓN La reciente eclosión de las neurociencias cognitivas ha generado un creciente interés por comprender las funciones y los sustratos neurales de las denominadas funciones cognitivas de alto nivel [1]. En las dos últimasdécadas, la neurología conductual y la neuropsicología han evolucionado a pasos agigantados bajo el influjo de los modelos teóricos provenientes de la psicología cognitiva, pero también por el avance de nuevos y sofisticados métodos que permiten estudiar la actividad cerebral durante los procesos cognitivos [2]. Así, las técnicas de neuroimagen y los modelos computacionales de las funcionescognitivas de alto nivel han arrojado nuevos datos y modelos sobre el intrincado mundo del funcionamiento cerebral. El córtex prefrontal es la región cerebral con un desarrollo filogenético y ontogénico más reciente y, por ello, la parte del ser humano que más nos diferencia de otros seres vivos y que mejor refleja nuestra especificidad; constituye aproximadamente el 30% de la corteza cerebral [3]. Desdeun punto de vista funcional puede afirmarse que en esta región cerebral se encuentran las funciones cognitivas más complejas y evolucionadas del ser humano; se le atribuye un papel esencial en actividades tan importantes como la creatividad, la ejecución de actividades complejas, el desarrollo de las operaciones formales del pensamiento, la conducta social, la toma de decisiones y el juicio ético ymoral [4,5].
