Six Sigma Project Report Template Quick Reference

General Requirements for Report:
Your report must follow the DMAIC outline and meet all minimum requirements identified below. Note: we recognizethe content, length and emphasis of individual sections within your reports will vary based on your actual findings and writing styles.

Required Section Content

The headings below must be included inyour report. The rest of the text below refers to content that you either must include in your report or may include. For more detailed explanation and examples of each of these sections, refer tothe Six Sigma Project Report Template.

1. Cover Page: Title, Name, E-mail, Company and Date of Submittal. In addition, students must identify a Project Mentor in their company (Supervisor, Qualitymanager, Six Sigma Black belt) who can authenticate and validate the findings in the report.

2. Executive Summary (Standalone document - 1 page maximum)
a. Problem Statement
b.Critical tools/ methods used in Analysis
c. Major results / findings
d. Conclusions & Recommendations

3. Improvement Opportunity: The Define Phase
a. Definition of the problem(restatement of problem and project scope).
b. Key measures used in evaluating success.
c. Impact on key business issues.

4. Performance: The Measure Phase
a.Identification of key input and output variables (KPIV's and KPOV's)
b. Quantify current performance level
c. Identify target performance level

5. Analysis & Findings: The Analyze Phasea. You must show evidence of the use of at least one data analysis tool/ method You should accomplish this task by including at least one table, graph, or diagram within the main body of the text.(e.g., include a Pareto chart, cause-and-effect diagram, scatter plot, etc. Note: Paste Excel graphs/tables as pictures into report to reduce file size.
