UNA INTERVENCIÓN BREVE EN PACIENTES DE CONSULTA EXTERNA

R ESUMEN

This is a report of the results of an observational study of patients
in brief psychotherapy treatment, with the following aims: a) To
compare the psychosocial characteristics of the patients whofinished treatment, with those that prematurely dropped-out, and
b) To assess the outcome of this treatment in patients that
completed it. The sample contained 111 patients, 83 women (74
%) and 28 men (28 %), who were referred by their psychiatrist to
the Psychotherapeutic Service of the National Institute of
Psychiatry Ramon de la Fuente. During the first eight weeks, the
following clinical andpsychosocial scales were applied to patients
attending this Service for the first time: personality inventory,
scale of psychiatric symptoms, social adjustment scale, scale of the
percibed social support, scale of ego strenght, and selfsteem scale.
From the 111 patients who requested attention, 25 did not come
for the initial interview, and 4 were considered as not suitable,
therefore the sample wasreduced to 82 patients (64 women and
18 men). The treatment consisted of 20 sessions of group
psychotherapy or 16 sessions one every two weeks, of brief
individual psychotherapy. Fourty-two patients (51%) completed
at least 16 treatment weeks, and 40 (49%) dropped-out at some
stage of the process. At the end, those who completed their
treatment were asked to answer once more the previously appliedscales. When comparing the initial scores of the patients who
completed their treatment (n=42), whith those of the drop-outs
(n=40) no significant differences were found, but when comparing
the final scores of the patients who completed the treatment, with
their initial scores, significative differences were found in various
scales, specially in anxiety, depression, hostility and social
adjustment.Suitable research methods to assess the complex
process implied in the psychotherapeutical treatment must be
found.

Se presentan los resultados de un estudio observacional de pacientes en tratamiento psicoterapéutico que tuvo los siguientes
objetivos: a) comparar las características psicosociales de los pacientes que permanecen en tratamiento con las de los que lo
abandonan en forma prematura, yb) valorar los efectos de dicho
tratamiento en aquellos que permanecen hasta concluir el proceso
terapéutico. Para el objetivo a) la muestra fue de 111 pacientes, 83
mujeres (74.3%) y 28 varones (26.3%) que acudieron referidos
por sus médicos tratantes al Servicio de Psicoterapia del Instituto
Nacional de Psiquiatría Ramón de la Fuente. Durante las primeras
ocho semanas, a cada paciente quesolicitó cita de inicio, se le
aplicó una batería de escalas clínicas y psicosociales compuesta
por: inventario de personalidad, escala de sintomatología
psiquiátrica, escala de ajuste social, escala de apoyo social percibido,
escala de Fuerza del Yo y escala de autoestima. De los 111 pacientes
que solicitaron cita, 25 no acudieron a la entrevista inicial y a 4 se
les consideró como no idóneos; por lotanto la muestra quedó
conformada por 82 pacientes, 64 mujeres y 18 varones. El
tratamiento consistió en una sesión de psicoterapia de grupo
semanal, durante 20 semanas o en psicoterapia individual breve:
16 sesiones, una por quincena. Cuarenta y dos pacientes (51%)
completaron al menos 16 semanas de tratamiento y 40 (49%)
dejaron de acudir a éste en alguna fase del proceso. Al finalizar, a
losque completaron dicho proceso se les aplicó la misma batería
de escalas. Al comparar las puntuaciones iniciales no se encontraron diferencias significativas entre los pacientes que permanecieron en tratamiento (n=42) y los que no tuvieron apego (n=40). La
