Factores predictivos para el diagnóstico temprano de coledocolitiasis
Predictive values for early diagnosis of choledocholithiasis

Resumen Objetivo: Determinar la utilidad de los factores predictivos paradiagnóstico de coledocolitiasis, basado en estudios clínicos y de laboratorio. Sede: Hospital de segundo nivel de atención. Diseño: Estudio prospectivo, observacional y comparativo. Análisis estadístico: Pruebas de tendencia central y de dispersión. La comparación entre los grupos se realizó por la t de Student y c2, dependiendo del tipo de variable. Se calculó sensibilidad, especificidad, valor predictivopositivo y valor predictivo negativo de la prueba diagnóstica. Material y métodos: Se realizó un estudio en el Hospital Regional PEMEX, Salamanca, Gto., en un periodo de enero a diciembre de 2007. Se incluyeron todos los pacientes con diagnóstico sugestivo de coledocolitiasis y se tomaron como parámetros de estudio el cuadro clínico (dolor, fiebre e ictericia), estudios de laboratorio(transaminasa glutámico oxaloacética, transaminasa glutámico pirúvica, fosfatasa alcalina, gamma-glutamil transferasa, bilirrubina total y bilirrubina directa) y ultrasonograma de vesícula y vías biliares, para descartar presencia de cálculos en vesícula biliar y en colédoco, así como dilatación de conductos biliares intra y extrahepáticos. Resultados: Se estudiaron 38 pacientes, 16 hombres (42.10%) y 22mujeres (57.89%), con una relación hombre/mujer de 1/1.3, la media de edad fue de 56 años (± 15.88). Se dividieron en 2 grupos; el grupo 1 (n = 28) con algoritmo positivo y el grupo 2 (n = 10) con algoritmo negativo. A todos los pacientes se les realizó colangiografía transoperatoria. Se encontró en 28 pacientes verdadero positivo, 0 falso positivo, 4 falsos negativos y 6 verdaderos negativos. Abstract Objective: To determine the usefulness of predictive factors for the diagnosis of choledocholithiasis based on clinical and laboratory studies. Setting: Second health care level hospital. Design: Prospective, observational, andcomparative study. Statistical analysis: Central tendency and dispersion tests. Groups comparison was made with Student’s t-test and c2, depending on the type of variable. We calculated sensitivity, specificity, positive and negative predictive values of the diagnostic test. Patients and methods: The study was performed at the Regional PEMEX Hospital, in Salamanca, State of Guanajuato, Mexico, in theperiod from January to December 2007. All patients with a diagnosis suggestive of choledocholithiasis were included. Study parameters were the clinical symptoms (pain, fever, jaundice), laboratory tests (glutamic oxaloacetic transaminase, pyruvic glutamic transaminase, alkaline phosphatase, gamma glutamyl transferase, total bilirubin and direct bilirubin), ultrasonograph of the gallbladder and bileducts to discard the presence of calculi in the gallbladder and the choledochus, as well as dilation of intra- and extra-hepatic bile ducts. Results: We studied 36 patients, 16 men (42.10%) and 22 women (57.89%), with a woman:man relation of 1:2, the mean age was 56 years (± 15.88). Patients were divided in two groups; group 1 (n = 28) with positive algorithm, and group 2 (n = 10) with negativealgorithm. All patients were subjected to transoperative cholangiography.
