RESUMEN
Fundamentos: se viene constatando una tendencia creciente de consumo de sustancias psicoactivas entre los pacientes que son hospitalizados por causa psiquiátrica. En esta investigacion pretendemos establecer la prevalencia y las caractetisticas del consumo reciente no terapéutico de tales su+ tancia\. mediante la detecciónen sangre y orina de los paciente\ psiqmátricos admitidos en el hospital, la influencia de ese consumo en la presentación clínica de los trastornos mentales y recomendar un criterio de indicación de dichas determinaciones. Método: determinación en orina de cannabinoides, anfetammas, opioides y cocaína. mediante el srstema ADX”’ y en sangre mediante el sistema REA’“’ para la alcoholemia, en lasadmisiones en la Unidad de Psiquiatría del Hospital General Universitario de Valencia durante los 6 primeros meses del año 1995. Los diagnósticos se han basado en la CIE- 10 en su versión para la investigación, y la influencia del consumo detectado de sustancias en la presentación clínica de los trastornos se ha fijado por mayoría entre tres psiquiatras experimentados. Resultados: la prevalencia delconsumo detectado de sustancias psicotropas es elevada (casi un tercio de las admisiones) y ese consumo influye en la mayor parte de tales admisiones (casi el 89%) agravando la clínica del trastorno mental al ingreso. Las variables asociadas con más frecuencia al consumo detectado son ser varón y ser hospitahzado como consecuencia de una intervención judicial. Conclusiones: Se sugiere ladeterminación rutinaria de sustancias susceptibles de abuso en las admisiones psiquiátricas. Palabras clave: Psicotropos. Etanol. Cocaína. minas. Cannabis. Opioides. Determining Recent Non-therapeutical Consumption of Psychotropic Substancesfrom Blood and Urine in Patiens Admitted to the Psychiatric unit ofa General Hospital in Valencia
Background: An increasing trend in the consumption of psychoactive substances has been observed in psychiatric patients who are hospitalised. The aim of this research is to estabhsh the prevalence and the characteristics of the recent and non-therapeutic consumption of these substances, by detecting their presente in the blood and urine of psychiatric patientsadmitted to hospital. the influente of such consumption on the clinical manifestations of the mental disorders and to recommend criteria for such a phenomenon. Method: Detection of cannabis, amphetamines, opiates and cocaine, using the ADX”” system. and in blood through the REA”’ system for alcohol detection, in patients admitted to the Psychiatric Unit of the General University Hospital in Valencia,during the first six months of 1995. The diagnoses were based on the CIE-10 in rts research version, and the influente of the consumption of xubstances detected in the clinical manifestations of the disorders was determined by the majority decision taken be three experienced psychiatrists. Results: the prevalence detected in the consumption of psychotropic substances is high (almost one third ofthose admitted), and consumption played a part in the admission of most of these patients (nearly 89%), aggravating the mental disorder problem when they were admitted.
