Benefit Description
With this plan, you can visit in‐network and out‐of‐network doctors; however, when you stay in‐network, you will generally save more. You do not need to designate a primary care physician and noreferral is required to visit a specialist. To find in‐network providers in your area, visit www.aetna.com/docfind. With this plan, there are no primary care physicians or networks. Instead, you may visit any provider that you wish, and the plan will pay a percentage, up to usual and customary, for the expenses that you incur.

Benefit Description
With this plan, you choose your own primary care physician who will be the primary caregiver for most of your routine medical care. When specialist care isneeded, your primary care physician will make referrals and provide authorization for you to see a specialist. With an HMO, you can enjoy one‐stop convenience of medical centers that offer on‐site pharmacies and laboratories. Services at Kaiser facilities are covered at 100% after your co‐pay*. However, there is no coverage outside of the HMO, unless there is an emergency. *Some exclusions mayapply. Check the Plan Summary for details.
