SUMMARY
Social changes observed in the last decades, –derived from variations in the demographic structure of the population with an increasein the absolute numbers of adolescents and persons from the third age with special vulnerabilities for substance abuse; recurrent economical crisis where problems derived from the increasing level of drug abuse among this group are added to those that result from poverty and lack of opportunities, and modernization and globalization as well– have affected the lives of persons in the individual,familial and social areas where drug abuse is immersed. To the traditional problem of alcohol abuse among middle-aged males, now females and adolescents are abusing alcohol imitating adult models where drinking and drunkenness are often linked. In spite of the change in social perception that associates tobacco use with health risks, Mexican adolescents, specially females, are smoking more. To thetraditional problem of inhalation of solvents with psychotropic effects among children and adolescents and of marihuana among youth and some groups of the adult population, an increasing use of cocaine is observed, a trend that includes children and the poor sector of the population. An increasing use of IV drug use, an epidemic outbreak of heroin in selected cities bordering the United States, andthe emergence of new drugs such as met-amphetamines, are now part of the drug subculture. The article describes some recent trends in drug use and changes observed from the 1970’s, and discusses the perspectives for the future regarding the social context. The relation between drug use, sexual risk behavior and delinquency is discussed. The association with accidents and different forms ofviolence, including suicide and intrafamily violence, is also analyzed. The macro determinants of the problem and their relation to drug use (globalization, economical development and inequalities, migration, changes in gender roles and family composition) are reviewed. The article finishes with an overview of the challenges for the future and research need.

RESUMEN
Los cambios sociales de las últimas décadas –derivados de variaciones en la dinámica y composición de la estructura demográfica de la población, en que hay, en números absolutos, mayor cantidad de adolescentes y de personas de la tercera edad; crisis económicas recurrentes a las que se suman problemas de pobreza agravados por el incrementoen el uso de drogas, y una acelerada apertura social y cultural frente al proceso de modernización y globalización– han afectado las vidas de las personas en los ámbitos individual, familiar y social en que interviene el abuso de sustancias. Al tradicional problema del abuso de bebidas alcohólicas entre los varones de edad media, ahora se han sumado las mujeres y los adolescentes que copian losmodelos adultos asociados con frecuencia al consumo y la embriaguez. Aunque ha habido un cambio en la percepción social del tabaco como producto adictivo y dañino para la salud, los adolescentes, principalmente mujeres, lo consumen cada vez más. Asimismo, al tradicional problema del uso de inhalables entre los menores y de mariguana entre los jóvenes y algunos grupos de la población adulta, seagrega ahora un consumo cada vez mayor de cocaína, que ha llegado incluso a los niños y a los sectores pobres de la población; en los adultos, este consumo se ha incrementado por vía intravenosa. Por otra parte, se ha detectado un brote epidémico de consumo de heroína en algunas regiones de la frontera norte y también han aparecido nuevas drogas como las meta-anfetaminas.
