URGENCIAS HOSPITALARIO

RESUMEN:
La afluencia de pacientes hacia los Servicios de Urgencias Hospitalarios (SUH) ha sufrido un incremento a nivel
mundial desde el siglo pasado. Esta demanda conlleva a lasobrecarga de los SUH acarreando una serie de
consecuencias negativas, tanto para los pacientes como para el Servicio. En parte, este incremento de visitas
se debe a las personas que utilizan el SUH de forma inadecuada. Este fenómeno ha puesto de manifiesto la
necesidad de implantar Sistemas de Triaje Estructurado.
La función de triaje consiste en clasificar a los pacientes que acuden a un SUH (antesde que reciban prestación
asistencial) siguiendo criterios de gravedad clínica que permita diferenciar el grado de urgencia.
Existen 5 niveles de Triaje: nivel I a nivel V, de mayor a menor gravedad. Cada nivel va a determinar el tiempo
óptimo entre la llegada y la atención médica; así, el tiempo de demora oscila entre al atención inmediata (nivel I)
hasta 240 minutos (nivel V).
Este estudio se hallevado a cabo en un Servicio de Urgencias Hospitalario, entre el 21 y 27 de marzo de 2011, en
horario de mañana y tarde de 8 a 22 horas. Es de tipo observacional descriptivo transversal y se ha obtenido
una muestra de n=406 casos.
Los objetivos de este estudio han sido determinar el número de pacientes que hace un uso inadecuado del SUH,
así como valorar si se cumplen criterios de calidad encuanto al tiempo de espera desde que los pacientes quedan
registrados en Admisión, pasan al Servicio de Triaje y son atendidos por personal facultativo. Además también se
recogieron otra serie de datos como sexo, edad, acceso por petición propia u orden facultativa, nivel de triaje y
horas registradas en Admisión, Triaje y el informe médico.

ABSTRACT:

The patients flow towards the Hospital Emergency Services (“SUH”) has suffered a world-wide rise since the last
century. This demand involves the overload ofthe “SUH” carrying a series of negative consequences as much for
the patients as for the service. In part, this rise in visits is due to the people who use “SUH” in an inadequate way.
This phenomenon makes it clear the need to implant Structured Triage Systems.
The function of Triage consists of classifying the patients who come to a “SUH” (before they receive medical
assistance) following clinicalseriousness criteria that allow to distinguish the emergency degree.
5 levels of Triage exist: level I to level V, from more to less seriousness. Each level will determine the best time
between the arrival and the medical assistance; thus, the time of delay oscillates between the immediate assistance
(level I) until 240 minutes (level V).
This research has been carried out in a Hospital EmergencyService, between the 21st and 27th March 2011, during
the morning and afternoon work shifts from 8.00 till 22.00. It’s the observational, descriptive and transversal type
and it’s based in a sample of 406 cases.
The objectives of this research have been to determine the number of patients who make an inadequate use of
“SUH”, and also to value whether quality criteria, referred to the timing ofwaiting since the patients are registered
in Reception (Admission), are called by the Triage Service and are assisted by a doctor or nurse, are fulfilled or not.
Besides, other data were also collected, like the gender, age, the access by own petition or medical order, level of
Triage and registered times in Reception, Triage and medical report.
