Prospective evaluation of the Alvarado Scale in the diagnosis of acute appendicitis

Abstract
Objective: To determine the usefulness and efficacy
of the Alvarado Scale for the diagnosis of acuteappendicitis, using clinical and laboratory tests.
Setting: Regional Hospital Salamanca PEMEX. Second
level health care hospital.
Design: Transversal, prospective, analytical study.
Statistical analysis: Sensitivity, specificity, positive and
negative predictive values of the diagnostic test.
Material and methods: The study was performed from
July 2007 to December 2008. Patientsof any age and
gender with a diagnosis of acute appendicitis were
included, patients were captured sequentially during
the study period. We obtained the scores according to
the variables of the Alvarado Scale, appendicectomy
was performed in all patients, and the result from the
histopathological report was obtained. The studied
variables were age, gender, time of evolution,migration of pain to the right inferior quadrant, anorexia,
nausea and vomiting, pain in the inferior right quadrant, rebound, increased temperature, leukocytosis
and neutrophilia.
Results: We studied 113 patients subjected to appendicectomy, there were 63 men (55.8%) and 50
women (44.2%), mean age was of 28 (± 12.6) years.
The Alvarado Scale was applied to the 113 patients
withthe presumptive diagnosis of acute appendicitis. Less than 4 points were obtained in 12 patients,
from 5 to 6 in 25 patients, from 7 to 10 in 76 patients.
The histopathological report was positive for acute
appendicitis in 92 patients (81.4%), and histologically
normal appendices were found in 21 patients (18.6%).

Objetivo: Determinar la utilidad y eficacia de la Escala
de Alvarado para el diagnóstico de apendicitis aguda,
utilizando pruebas clínicas y de laboratorio.
Sede: Hospital Regional Salamanca PEMEX. Hospital
de segundo nivel.
Diseño: Estudio transversal, prospectivo y analítico.
Análisis estadístico: Sensibilidad, especificidad,valor
predictivo positivo y valor predictivo negativo de la
prueba diagnóstica.
Material y métodos: Se realizó un estudio en el periodo
de julio de 2007 hasta diciembre de 2008, se incluyeron pacientes de cualquier edad y género con diagnóstico de apendicitis aguda, captados secuencialmente en el periodo de estudio. Se obtuvo el puntaje
de acuerdo con las variables de la Escala deAlvarado,
se realizó apendicectomía en todos los pacientes y se
obtuvo el resultado del reporte histopatológico. Las
variables estudiadas fueron: edad, género, tiempo
de evolución, migración del dolor a cuadrante inferior derecho, anorexia, náuseas y vómitos, dolor en
cuadrante inferior derecho, rebote, elevación de la
temperatura, leucocitosis y neutrofilia.
Resultados: Seestudiaron 113 pacientes que fueron
sometidos a apendicectomía, hubo 63 hombres
(55.8%) y 50 mujeres (44.2%), la media de edad fue de
28 (± 12.6) años. La Escala de Alvarado fue aplicada
a los 113 pacientes con diagnóstico presuntivo de
apendicitis aguda, se obtuvieron menos de 4 puntos
en 12 pacientes, de 5 a 6 puntos en 25 pacientes, de 7
a 10 puntos en 76 pacientes. El reportehistopatológico fue positivo para apendicitis aguda en 92 pacientes (81.4%), y apéndices histológicamente normales
