1ERA: SEXUAL ABUSE IN CHILDREN OR ADOLESCENT

2DA:INTRODUCTION
Sexual abuse in children is becoming more frequent, or perhaps more frequently reported. We also know that sexual abuse is committed by any individual to whom the child or adolescent has known before, and we know that this occurs between 75 and 80% of cases and in 50% of cases are related, such as parents, whether biologicalor stepfathers, boyfriends, mothers, uncles, grandfathers, brothers, sisters. Are more likely to suffer child abuse outside the home by non-family members, including coaches, carers and teachers. But other authors reported that sexual abuse in childhood are caused by a relative (incest). Victims of sexual abuse come from all socioeconomic groups and all races. Approximately 75% of victims aregirls and 25% are children. But some researchers believe that the statistics regarding children are erroneously low, because children are less communicative than girls.

3ERA: DEVELOPMENT
 What do you do in case of sexual abuse and the consequences are expected?
It is better that the interview be conducted by an experienced professional in interviews and sexual abuse. This could be a doctor, asocial worker, psychologist or counselor for rape.
The interview should be such as to include open questions as: What happened? Had something similar happened? If so, when was the first time? Who did it? Anybody else have you done? How many times has happened? Where are the other people (eg., Your mother) when this happened? Did you tell anyone what happened? , Yes, Who? And what did they do? .Still others use helper methods as illustrations or dolls with anatomical details, but that it must decide the expert. Anything you say the child or the child, especially if they are statements, must be recorded in the history of the most literal way possible.
From there it passes the physical examination: It is very important that the examiner is familiar with normal anogenital anatomy of theprepubertal children.
Remember that in the male anatomy remains constant throughout childhood, with the exception of the increase in penis size and testes and the appearance of pubic hair during adolescence.
In contrast, the girls there are constant changes between infancy and adolescence. All girls are born with a hymen, which is thick and full and the hymen covers the opening in the newborn. Asreturning maternal hormones, the hymen becomes thin and translucent, often crescent-shaped. At the onset of puberty, the hymen thickens again, has a scalloped edge, and is full. The penetration of the hymen in postpubertal teenagers' can lead to strain without tearing tissue. In prepubescent girls, the cuts may heal leaving little residual evidence.
Physicians should be aware that a normalanogenital examination does not preclude the possibility of sexual abuse. Many types of abuse (such as genital contact and petting), not expected to result in visible changes.
In recent sexual abuse case (acute) may be readily visible evidence of injury, especially if you perform a scan with the aid of magnifying lenses, such as a colposcope, for their potential to produce images with augmentation, anddocumentation photographic or video tape.
Most recent genital lesions are located between the position of the 4 and 8 hours.
First injured the labia minora and the posterior fork, followed by tears of the posterior ring of the hymen. The hymen orifice measured in horizontal direction while the lips are separated laterally.
During the first 5 years, a horizontal diameter greater than 5 mm. isabnormal and should arouse suspicion of vaginal penetration.
Between 5 and 9 years, supported 1 mm. more per year. It is also unusual that the diameter greater than 9 mm. any child of 9 years and older who have not entered puberty (Tanner stage 2).
It is more difficult to evaluate children who have been abused in the past or chronic.
