AGREEMENT TO EXTEND LEASE AGREEMENT

THIS AGREEMENT made this ___ day of ________ by and between FIELD CAPITAL, LLC. (“Landlord”) and (“Tenant”).

WITNESSETH:

WHEREAS, theparties hereto entered into a Lease Agreement, dated _________________, (the “Lease”), for certain premises described as 736 N. Magnolia Avenue, Studio building, Orlando, Florida, and all improvementsthereon (“Premises”).

WHEREAS, the Lease term will expire on ____________ and the parties hereto desire to extend the term of the Lease as set forth below.

NOW, THEREFORE, for good andvaluable consideration, the receipt and sufficiency of which are hereby acknowledged, the parties hereto agree as follows:

1. Term of Extension. The Lease is hereby extended for a period of_________, commencing ______________, and expiring __________ (the “Extended Term”), at the same terms existing under the Lease as if this Extended Term had been part of the initial Lease Term asspecifically provided herein.

2. Rent Payments. Commencing on ____________, Tenant agrees to pay to Landlord an annual base rental rate (“Rent”) of _________ per annum payable, without setoff ordemand, in advance monthly installments of _________ to be paid each month on the first day thereof. Tenant further agrees to pay, together with rent all applicable sales tax, if any, on the rentand other payments to be paid to the Landlord under the Lease, as set forth in Section 2 of the Lease.

3. Continuing Effect. All of the terms and conditions of the Lease, as modified by thisAgreement to Extend Lease Agreement, shall remain in full force and effect. Tenant covenants and agrees that it continues to be bound by, and will perform and observe, all of the covenants, agreements,stipulations, and conditions on the part of the Tenant contained in the Lease

4. No Defenses, Claims or Set-offs. Tenant acknowledges that it is obligated to the Landlord according to the...
