PARATIROIDECTOMÍA INCIDENTAL EN CIRUGÍA DEL TIROIDES

RESUMEN
Antecedentes: La tiroidectomía es un procedimiento quirúrgico seguro. La principal complicación
posoperatoria incluye la lesión de las glándulas paratiroides.
Objetivos: Investigar la incidencia, factores de riesgo, relevancia clinica de la paratiroidectomía incidental.
Ambiente: Servicio de cirugía general.
Diseño: Estudio descriptivo retrospectivo no randomizado.
Población:168 pacientes sometidos a cirugía de la glándula tiroides.
Método: Durante el período octubre 1984 a agosto 1999, 168 pacientes se asignaron a dos grupos:
“A” resección incidentalmente de las paratiroides, y “B” sin resección. La tiroidectomía total o casi total
fue el procedimiento de elección; la lobectomía total y la subtotal contralateral fue realizada en cuatros
pacientes. La diseccióncentral de los ganglios linfáticos se realizó en cuatro pacientes con linfadenopatías
cervicales.
Medidas de evaluación, presencia de enfermedad tiroidea subyacente, número y tamaño de las
paratiroides resecadas, localización. Niveles de calcio sérico, medidos el día de la cirugía, y en los 1, 2,
y 7 o más. Diagnóstico clínico e histológico de enfermedad benigna versus maligna, tipo de cirugía.Resultados: El porcentaje de pacientes del sexo femenino en el grupo “A” fue significativamente
más alto que en el “B” (93% vs. 58,5%, p = 0.0002). No se evidenció diferencias significativas con respecto a la ocurrencia de hipocalcemia postoperatoria clínica y bioquímica entre los dos grupos
(p = 0.33).
Conclusiones: La paratiroidectomía incidental no es infrecuente durante la tiroidectomia.Un alto
porcentaje de casos se deben a la localización intratiroidea. Debe ser considerada como una complicación clínicamente menor.

SUMMARY
Background: Thyroidectomy is a relative safe surgical procedure. The main postoperative
complications include injury to the parathyroid glands.
Objective: To investigate the incidence, risk factors and clinical relevance of incidentalparathyroidectomy during thyroid surgery.
Setting: General Service university hospital.
Design: Study retrospective no randomized.
Population: 168 patients who underwent thyroid surgery during 1984 to 1999.
Method: The clinical records were reviewed. Pathology reports were carefully reviewed for the nature
of the underlying thyroid disease. Two groups of patients were studied: a, group with incidentalparathyroidectomy, and b, without incidental resection. Total/near-total Thyroidectomy was the procedure
of choice. Total lobectomy and contralateral subtotal lobectomy was performed in the other 4 patients.
Elective central neck lymph node dissection was performed in four patients with neck lymphadenopathy.
Outcomes measures. Underlying thyroid disease, presence, number and size of incidentallyresected
parathyroid gland(s), their location and possible parathyroid pathology. Serum calcium levels, were
measured preoperatively, on day of surgery, and on postoperative days 1, 2 and 7 or even later as needed.
Results: Inadvertently removed parathyroid tissue was found in 17,7%. The percentage of women
in group "A" was significantly higher than in group "B" (93% vs 58,5%, p = 0.0002).
Therewere no statistically significant difference regarding the occurence of postoperative hipocalcemia
betwen the two gropus (p = 0.33).
Conclusions: Incidental parathyroidectomy is not uncommon following Thyroidectomy. QA significant
percentage of cases it may be due to the intrathyroidal location of the parathyroid. Incidental
parathyroidectomy may be considered as a potentially preventable....
