Places: TPG – Inarpi’s Port TerminalThis is to certify that the undersigned surveyor from VERIFICADORES ECUATORIANOS S.A. “VERIFECSA”, according to the instructions received from ECUAPANDI S.A., local Correspondents of CONOSUR – COMPAÑIA DE SEGUROS Y RE-ASEGUROS, carried out a survey to a Harvester Machine discharged from M/V “SEABORD AMERICA” V.007 SB, on August 17th 2011


1. SCOPE OF THE SURVEY

This survey wasrequested by INARPI S.A. with the purpose to ascertain in the condition of a Harvester Machine consigned to Mr. OTTO HAON PIEDRAITA, same that suffered an accident during discharging operation carried out at TPG’s Port Terminal of Guayaquil

3. Chronology

* August 21st , 2011
The attending surveyor was present at TPG-INARPI’s premises – Module # 1, in order to establish the cause and extension of damages caused to the harvester machinefrom 09:30 hrs until 10:30

* August 23rd - 24th , 2011
The attending surveyor was present at TPG-INARPI’s premises – Module # 1, during the mobilization process from 17:00 hrs until 18:00 hrs & from 09:00 until 12:30 hours respectively

* August 30th, 2011
The attending surveyor was present at TPG-INARPI’s premises – Module # 1, during the mobilization process from 09:30hrs until 16:00 hrs.

5. PRECEDENTS

As per information obtained during our survey, on 17th August, 2011 at approximately 20:40 hours during the unloading operations carried out at the port terminal of Guayaquil - TPG, the above described machine, with D/R # 1777 suffered damages in its lower rear structure, apparently caused due to an apparently bad maneuver carried out by the crane´soperator and/or failure of the lashing devices, used during the discharging operation , same that caused the collapse of the structure, when the machine was being discharged at the pier and approximately from 2 meters high abruptly fell to the ground.

6. FINDINGS

As per instructions received from INARPI S.A., through ECUAPANDI S.A., P & I Local Correspondents of Conosur-Compañia deSeguros y Reaseguros – Chile, on 21st August, 2011, at approximately 09:15 hours, a delegate of our company attended at TPG/Inarpi, Module # 1.

The harvester machine was found to be temporarily placed at the central access to the piers. Several wooden pieces were placed underneath of it to support the rear section of the lower structure of the machinery. The following structural damages...
