PURPOSE OF REPORT: The person named is applying to the Foreign Language Teaching Assistant Program at a college or university in the United States. Consideration must be given toeach applicant’s proficiency in the language of nomination. This report form seeks a reliable evaluation of the applicant’s present command of the language for which they are being nominated.Please indicate briefly how the evaluation was conducted.
