(CHILD)

I/We, the undersigned parents, or legal guardians of Santiago San Romàn (“Child”) hereby consent to the Child’senrollment in the Jim McLean Golf School Junior Program (“Program”) to be held at Doral Golf Resort & Spa (“Club”), and acknowledge and agree as follows:

1. I/We understand that the Child will participatein a number of sports activities including without limitation, golf, tennis and swimming while attending the Program. I/We understand that these activities will involve physical exertion by the Childas well as contact with other children attending the Program, and that many of the activities will take place outdoors.

2. I/We understand that all possible precautions are taken to assure that theactivities of the Program are conducted by mature and qualified personnel in a safe and responsible manner. However, because of the nature of some activities, I/We understand that there is apotential risk for bodily injury, property damage and/or death occurring to the Child arising either directly or indirectly out of the Child’s participation in the Program. I/We do recognize these risks andallow the Child to participate in the activities of the Program, and, as a material inducement to the Club, the Club owner Pyramid LLC and Jim McLean Enterprises, Inc. to allow the Child’sparticipation in the Program, I/We hereby release and agree to hold harmless Club, Pyramid LLC, and Jim McLean Enterprises, Inc. and all successors, assigns, affiliates, partners, co-venturers, shareholders,directors, officers and employees of the foregoing (all the foregoing are hereinafter referred to as “Sponsors”), from and against any and all manner of actions, causes of action, liabilities,controversies, agreements, promises, damages, rights, injuries, judgments, executions, claims and demands of any nature whatsoever at law or in equity, that I/We and/or the Child may have now or in the...
