This form is to be used by SUBWAY® Franchisees and Development Agent offices only. If you are a SUBWAY® Franchiseewho wishes to be involved in another food business while you are still a SUBWAY® Franchisee or after your SUBWAY® Franchise Agreement has transferred, expired, or terminated you may submit this formon behalf of yourself. If you are a Development Agent office you may submit this form on behalf of a current SUBWAY® Franchisee who wishes to be involved in another food business while they are still aSUBWAY® Franchisee or after their SUBWAY® Franchise Agreement has transferred, expired, or terminated. Development Agent offices may also submit this form on behalf of a prospective SUBWAY®Franchisee who currently owns another food business. The SUBWAY® Franchise Agreement has in-term and post-term non-compete provisions which prevents SUBWAY® Franchisees from having any direct or indirectassociation with a Competitive Business during or after the term of their SUBWAY® Franchise Agreement which is identical to or similar to a SUBWAY® Restaurant. "Competitive Business" shall mean anybusiness that sells any type of sandwiches on any type of bread, including but not limited to: sub rolls and other bread rolls, sliced bread, pita bread, flat bread, croissants, English muffins and bagels.Sandwiches do not include hamburgers or hot dogs. Exempt from this provision are full service restaurants where customers are served by wait staff and pay after eating. Please fill out the following:1.
