Wedding Questionnaire
• Please, provide your full names, both English and Hebrew names (if applicable).
• List the names by which you would like to be addressed at the ceremony.
• List thenames of all close family members who will attend.
• How many people are expected to attend?
• How long do you wish the ceremony to last? (15 min to 1+ hour, average of 30min)
•What tone would you like for the ceremony? (Intimate, ceremonial, minimal, sacred, warm, solemn, lighthearted?)
• Do you want a spiritual, religious, humanist, or personal service (or acombination thereof)?
• Are there any special readings, prayers, etc you would like to include?
• What do you definitely want and definitely do not want for your ceremony?
• Whatwould you like to convey to all in attendance?
• Do you wish to include, mention or honor any family members or friends? If so, who?
• Do you want to honor/mention anyone who has passedaway?
• Will you write your own vows? You can whether use standard vows, or write your own. Just let me know in advance.
• Do you have a bridal party? How do you picture the processional/recessional?
• Witnesses must be appointed for the Ketubah signing. And I would need their names. They cannot be blood relatives or in-laws to either of you.
Backgrounds and Families:
• What are your religious, cultural, and family backgrounds? Where were you raised?
• Are you close to your families? Please elaborate.
