Guide for Completing Form AB-40


1. PURPOSE


To provide guidance for completing Form AB-40, Boiler and Pressure Vessel Repair and Alteration Report.


2. APPLICATION


This report is used for documenting repairs, alterations, and rerates for boilers, pressure vessels, thermal liquid heaters, direct and indirect fired heaters under the Safety Codes Act and Regulations.Piping repairs are to be documented on Form AB-83, Construction Data Report for Piping Systems.


3. GENERAL


Form AB-40 is required for documenting repairs and alterations and for certifying that the repair/alteration was done in accordance with Alberta requirements. Any welding on the pressure envelope or any change in the pressure and/or temperature rating must be documented onForm AB-40 (the latest version shall be posted on ABSA's website http://www.absa.ca under ABSA Forms).


Repair and alteration procedures are to be submitted to ABSA Safety Code Officer (SCO) for acceptance prior to start of work. Note - Alterations and repairs must be submitted to ABSA Design Survey when required by ABSA SCO. Details of the accepted repair or alteration and verification ofacceptance must be at the worksite for final inspection.


Alteration - Any change in the item described on the original Manufacturer's Data Report, which affects the pressure containing capability of the pressure retaining item. Non-physical changes such as a change in the maximum allowable working pressure (internal or external) or design temperature of a pressure retaining item is analteration. A reduction in minimum design metal temperature is also an alteration.


Repair - The work necessary to restore pressure equipment to a safe and satisfactory operating condition, provided that there is no deviation from the original design.


4. INSTRUCTIONS


The following describes the information required for completing each item of Form AB-40. The headings ornumbers in this instruction correspond to the heading and numbers on Form AB-40. Highlighted areas on the AB-40 form also show the instructions.


Where 'repair' is used this may also mean alteration.
Side A
(A)# - provide Alberta Identification Number assigned by ABSA SCO.
Owner Equipment Number - provide vessel identification number when assigned by owner.
Repair and/oralteration – check one or both as applicable.
Partial - check box if this is part of a repair/alteration which will be completed by another repair organization. Form AB-40 must be completed and certified and provided to the final repair organization.
Final - check box if this is the report for the completed repair/alteration.
1. Identify repair organization name and addressProvide AQP number and expiry date – ABSA approved quality program number (1000, 2000, or 3000 series) of the organization doing the actual repair.
Provide location of installation - LSD, street address, and/or facility name
2. Provide name and address of owner of the boiler or vessel.
3. Description - what the owner calls the pressure equipment. Provide name of manufacturer, CRN (CanadianRegistration Number), and manufacturer’s serial number
4. Provide design conditions – as taken from nameplate. Indicate units of measure (psig or kPa; (F or (C). For shell and tube heat exchangers or multipart vessels all design conditions are to be included. Shellside and tubeside are for shell and tube exchangers; jacket is for jacketed vessels.
5. Provide a description of the defect thatresulted in the repair eg:
Location: Internal/external, in nozzles, heads etc.
Type: General corrosion
Pitting, weld decay
Cracking
Other
6. Indicate which ASME code edition, year and addenda is used for the repair. The design may be accepted based on the original or latest code of construction but repair must be to...
