MONITORIZACIÓN DOMICILIARIA DE PACIENTES PORTADORES DE DISPOSITIVOS CARDÍACOS IMPLANTABLES: ACTIVIDADES DE ENFERMERÍA EN LA ATENCIÓN DE ESTOS PACIENTES

Resumen • Introducción: En los últimos años ha aumentado la esperanza de vida y es preciso implantar un mayor número de marcapasos en la tercera edad, mayor número de desfibriladores, especialmente como prevención primaria y resincronizadores en una población más numerosa depacientes con insuficiencia cardiaca avanzada y la consecuencia ha sido la sobrecarga de trabajo en las Consultas de Seguimiento. La creciente complejidad tecnológica de los dispositivos aumenta el tiempo empleado en su control y seguimiento, también contribuye a esta sobrecarga la presencia de alertas de seguridad sobre dispositivos y cables. Además, pacientes y familiares demandan más facilidades yatención continuada. El seguimiento remoto puede ofrecer una alternativa práctica para el profesional y cómoda para el paciente, sin perder seguridad y calidad asistencial. • Objetivo: Con este artículo queremos comunicar nuestra experiencia en la implementación de la plataforma tecnológica Home Monitoring de Biotronik a la estructura de la consulta, el papel que enfermería desarrolla en estamodalidad de seguimiento y la adaptación de los pacientes a esta nueva tecnología, así como revisar los aspectos fundamentales de la monitorización domiciliaria y las actividades de enfermería en la atención a estos pacientes. • Conclusiones: El seguimiento remoto es fiable y fácilmente aceptado por pacientes y profesionales. Para enfermería nace una nueva actividad para la cual debemos fomentar eldesarrollo profesional, así como potenciar y ejercer nuestra faceta educadora y cuidadora. REMOTE MONITORING FOR PATIENTS WITH IMPLANTABLE CARDIAC DEVICES: ACTIVITIES OF NURSING CARE OF THESE PATIENTS Abstract • Introduction: Over the last years life expectancy has increased and a major number ofpacemakers are therefore implanted in the elderly population. Also a major number of ICDs, especially in primary prevention and a major number of CRT devices are implanted in an increasing population of advanced heart failure patients. The consequence of all this is an increase of the workload in the follow-up centers. The increasing technological complexity of the implanted devices increase the timespent in patient check-ups and follow-ups. The different patient options for the devices and electrodes also contribute to the increase of the workload. On the other hand, patients and relatives require more continued service over time. The remote monitoring can represent an alternative, useful for the health professionals and comfortable for the patients, ensuring the same level of safety andhealthcare quality. • Objective: With this article, we want to transmit our experience in implementation of the technological platform BIOTRONIK Home Monitoring into the structure of our follow-up centre, the role of nursing in the follow-ups and the patients’ adaptation to this new technology. In this article we will also review the fundamental aspects of remote monitoring and of the patientmanagement activities deployed by nursing. • Conclusions: remote monitoring is a reliable system, easily accepted by patients and health professionals. This is a new activity for the nursery sector, and we should therefore strengthen our professional skills and promote the educational and care provider aspects of our profession.
