Utilidad de la tomografía multicorte en la valoración del paciente con cólico renoureteral

Urinary stone disease is a frequent illness, being the acute uretral obstruction the cause of consultation of first time, manifested as renal colic. The value of the image studies in these patients resides in that near 90% of stones are visible on plain films. Intravenous Urography has been used during a lotof time for the evaluation of stone disease. The recent development of the computed tomography (CT) has allowed an appropriate valuation of the stones and other possible causes of flank pain. The multidetector-row CT systems allow to value the genitourinary tract after the administration of contrast media. Material and methods. Ten patients were studied with renal colic and they were carried outcomputed tomography urography (CTU) in a 16-detector row CT. They were carried out three study phases: Native phase (nonenhanced phase), nefrographic phase (70100 seconds post-injection of the contrast) and pielographic phase (200-240 seconds post-injection). Results. Renal stones was detected in five patients, eight patients presented ureteral stones. All the stones were observed in the nonenhancedphase. Five patients with ureteral stones presented tomoraphic signs of acute ureteral obstruction, two patients presented data of acute inflammatory process. Other findings included cystic lesions in annexes, thickness of the seminal glands and fleboliths coexisting with ureteral stones. Conclusions. The intravenous urography in our country is the main image study for the patient’s evaluationwith flank pain and renal colic. However, the computed tomography in a nonenhanced phase demonstrates the cause of renal colic almost always. The multidetector row CT is useful in the evaluation of patient with renal colic, but they should be considered several factors, among them the radiation dose, an then be able to decide the best image method for our patient. RESUMEN Introducción. La litiasis renal es una enfermedad frecuente, siendo la obstrucción ureteral aguda la causa de consulta de primera vez, manifestada como cólico renoureteral. El valor de los estudios de imagen en estos pacientes radica en que cerca de 90% de los litos son radiopacos. La urografía excretora se ha utilizado durante mucho tiempopara la evaluación de la litiasis. El rápido desarrollo de la tomografía computada ha permitido una adecuada valoración de los litos y otras posibles causas de dolor. Actualmente los equipos de tomografía multicorte permiten valorar la vía urinaria posterior a la administración endovenosa de medio de contraste. Material y métodos. Se estudiaron 10 pacientes con cuadro clínico de cólico renoureteraly se les realizó estudio de urografía por tomografía computada en un tomógrafo helicoidal multicorte de 16 detectores. Se realizaron tres fases de estudio: Fase nativa (rastreo en fase simple), fase nefrográfica (70-100 segundos postinyección del contraste) y fase pielográfica (200-240 segundos post-inyección). Resultados. Se detectó litiasis renal en cinco pacientes, ocho pacientes presentaronlitiasis ureteral. Todos los litos sin excepción se observaron desde la fase simple. Cinco pacientes con litiasis ureteral presentaron datos tomográficos de obstrucción ureteral aguda, dos pacientes presentaron datos de proceso inflamatorio agudo.
