Abordaje psicopedagógico del trastorno por déficit de atención con hiperactividad con el modelo de entrenamiento de las funciones ejecutivas

PSYCHOPEDAGOGICAL MANAGEMENT OF ATTENTION DEFICIT HYPERACTIVITY DISORDER WITH THE MODEL OF EXECUTIVE FUNCTION TRAINING Summary. Different therapeutic models have been presented withwhich to treat the condition of attention deficit hyperactivity disorder (ADHD) and the scientific literature speaks of the benefits linked to multimodal approaches. The National Institute of Mental Health report grants combined treatments an essential role. Throughout the literature authors agree that the executive functions are intrinsically related to the functions of the frontal lobe. Weconsider executive function training (EFT) to be another resource available within the different approaches used to treat ADHD and which is oriented towards developing and reinforcing the neuropsychological functions so as to give rise to new cognitive resources. Working memory is responsible for storage for a minimum amount of time while the information needed to resolve higher cognitive processes ismanipulated. Some authors consider executive dysfunction to be a distinguishing element of ADHD. This type of dysfunction would make it more difficult for the higher cognitive processes to be carried out and is therefore a disorder that conditions the problems observed in the formal academic learning processes. The EFT programme consists of specific work modules and is aimed at favouring theappearance, development and operation of the affected functions. [REV NEUROL 2004; 38 (Supl 1): S149-55] Key words. INTRODUCCIÓN La condición del trastorno por déficit de atención con hiperactividad (TDAH) muestra hoy un interés relevante en el campo científico y ocupa un lugardestacado tanto en la clínica como en el ámbito de las neurociencias. En las ultimas décadas, el diagnóstico de los diferentes subtipos clínicos y neuropsicológicos, así como el tratamiento de este trastorno con sus diversos modelos terapéuticos, han estado en constante revisión. En la actualidad, la literatura científica habla de los beneficios asociados a los abordajes multimodales. El informe delNational Institute of Mental Health (NIMH) asigna a los tratamientos combinados un papel esencial. La experiencia clínica del TDAH muestra que la intervención multimodal ofrece resultados favorables en la población afectada. Este tipo de abordaje debe considerar, dentro de sus objetivos generales, no sólo el aspecto farmacológico, sino también el contexto psicosocial del niño y su familia desde unmodelo integral. Paralelamente se implementan, como objetivos más específicos, estrategias orientadas a desarrollar y potenciar funciones neuropsicológicas afectadas, a modo de compensar las mismas y permitir nuevos recursos cognitivos. El entrenamiento de las funciones ejecutivas tiene como objetivo favorecer en cada paciente, a la luz del diagnóstico neuropsicológico, aquellas funciones que hanresultado afectadas. El programa de entrenamiento de las funciones ejecutivas (EFE) constituye un procedimientos más dentro del abordaje multimodal del TDAH.
