ACHANTOSIS NIGRICANS AND HYPERINSULINEMIA
IN OBESE CHILDREN AND ADOLESCENTS

RESUMEN

obese children and adolescents who were referred
to the Pediatric Endocrinology Departmentof the
Instituto Nacional de Salud del Niño Lima - Peru
from January 2001 to December 2003. Design:
descriptive, retrospective study. Material and
methods: there were revisited 1 601 charts from
obese children and adolescents referred to the
Pediatric Endocrine Department from January 2001
to December 2003. Results: from 1601 obese
patients, 197 (12,3%) showed AN, serum insulin
wasdetermined in 63 (32%). Thirty nine (19,8%)
patients had AN plus hyperinsulinemia, 56,4% were
girls, 59% were more than eleven years of age and
33,3% were pre puberal. The median of body mass
index (BMI) was 29,4 Kg/m 2. There were familiar
history of Diabetes Mellitus Type 2 (DMT2) in
51,3%, HTA in 15,4%, and parental obesity in
35,9%. One hundred and twenty one patients
(61,5%) showedhypercholesterolemia. Achantosis
isolated was found in 24 (12,2%). Conclusions: from
197 obese patients with AN, serum insulin was
determined in 63; thirty nine (62%), showed
hyperinsulinemia, signaling the high association
among between hyperinsulinemia and obesity with
AN; this result remarks the necessity to determine
serum insulin in obese patients with AN, for early
detection of risk todevelop DM and other metabolic
consequences.

Objetivo: determinar la incidencia de acantosis
nigricans (AN) e hiperinsulinemia en niños y
adolescentes obesos atendidos en el Servicio de
Endocrinología del Instituto Nacional de Salud del
Niño (INSN) durante el periodo 2001-2003. Diseño:
estudio descriptivo, retrospectivo. Material y
métodos: se revisó las historias clínicas de 1 601
pacientesobesos que acudieron por primera vez al
consultorio externo del Servicio de Endocrinología
del INSN durante el periodo de enero del 2001 a
diciembre del 2003. Resultados: de 1 601 pacientes
obesos, 197 (12,3%) presentaron AN, de los cuales
116 (59%) fueron niños mayores de 11 años y 33,3%
estaban en estadio prepuberal. Se determinó insulina
a 63 (32%). Treinta y nueve pacientes (19,8%)presentaron AN más hiperinsulinemia, 56,4% de
éstos fueron de sexo femenino. La media del índice
de masa corporal (IMC) fue de 29,4 Kg/m 2 . Se
encontraron antecedentes familiares de Diabetes
mellitus tipo 2 (DMT2) en 51,3%, hipertensión
arterial (HTA) en 15,4% y obesidad en 35,9%. Se
evidenció hipercolesterolemia en 121 pacientes
(61,5%). La acantosis aislada se encontró en 24
(12,2%).Conclusiones: de 197 pacientes obesos con
AN se determinó insulina en 63, se evidenció
hiperinsulinemia en 39 (62%). Ello muestra la alta
asociación entre hiperinsulinismo y obesidad con
AN; siendo necesario realizar determinaciones de
insulina en pacientes obesos con AN, a fin de detectar
tempranamente a los que tienen riesgo de desarrollar
DMT2 u otras alteraciones metabólicas.

INTRODUCCIÓN
La Acantosis Nigricans (AN) es una
dermatosis que está clínicamente caracterizada
por una hiperpigmentación y engrosamiento
cutáneo de aspecto aterciopelado 1-3, ha sido
reportada por algunos como resultadode un
incremento de melanocitos y melanina 4 ,
mientras otros creen que probablemente está más
relacionada al espesor del contenido de los
