Hipertensión arterial en pediatría. Tratamiento basado en su
fisiopatología

RESUMEN
Antecedente. El tratamiento de la hipertensión arterial (HTA) guiado por la prueba de renina (Renin-Guided Treatment ofHypertension
RGTH en inglés) es un algoritmo valioso, que toma en cuenta los subtipos fisiopatológicos de esta enfermedad. Algunos investigadores
sugieren que se incluya sistemáticamente esta prueba como una guía útil para el tratamiento de la HTA; sin embargo, no hay estudios
de su eficacia en la población pediátrica.
Objetivo. Obtener datos retrospectivamente con objeto de saber si se justificarealizar un estudio prospectivo para determinar la eficacia
del algoritmo mencionado en niños con HTA.
Material y método. Estudio histórico comparativo de cohorte basado en los datos de expedientes de niños hipertensos estudiados en el
Instituto Nacional de Pediatría (INP). Se describió y se comparó el tiempo necesario para normalizar la presión en dos grupos de pacientes;
unos tratados con elalgoritmo RGTH (Grupo en estudio) y otro tratado con el método convencional (Grupo control). Se identificaron las
variables que influyen en el tiempo requerido para revertir la HTA.
Resultados. Hubo 39 pacientes en el Grupo en estudio y 43 en el Grupo testigo. Ambos fueron similares en edad, presión arterial sistólica
y diastólica. La mediana del tiempo transcurrido para normalizar la presión en elGrupo en estudio fue 44 horas (IC 95%: 40-50 horas) y
en el Grupo testigo, 89 horas (IC 95%: 72-82 horas). Esta diferencia fue altamente significativa (Log-Rank: P=0.0008).
Conclusiones. Se justifica un estudio prospectivo para determinar la eficacia del algoritmo de RGTH en niños con hipertensión arterial.

ABSTRACT
Background. The treatment of arterial hypertension (AHT) based on the rennin level (Renin-Guided Treatment of Hypertension RGTH)
is a valuable algorithm based on the physiological subtypes of this condition. Some authors suggest that the rennin level test should be
included routinely as a useful guide for the treatment of AHT. However there is no information of its efficacy inpediatric practice.
Objective. Obtain retrospective information to decide whether a prospective study is justified in order to determine the efficacy of the
above algorithm in children with AHT.
Material and method. A study of a comparative historic cohort based on the charts of hypertensive children studied at the Instituto Nacional
de Pediatría (INP) was done. We recorded and compared the time requiredto normalize blood pressure in two groups of patients: one
group (study Group) was treated following the RGHT algorithm. Another group (control Group) was treated with the conventional method.
Variables influencing the time for control of AHT were recorded.
Results. There were 39 patients in the Study Group and 43 in the Control Group. Both groups were similar in age, and in systolic anddiastolic arterial blood pressure. The time median to normalize arterial blood pressure in the Study Group was 44 hours (CS 95%: 40-50
hours) For the Control Group it was 89 hours (CS 95%: 72-82 hours). This difference was highly significant (Log-Rank: P = 0.0008).
Conclusions. A prospective study to determine the efficacy of RGHT algorithm in hypertensive children is justified.
