GUIDELINES FOR USING YOUR SCHOLARSHIP AWARD Please review all information carefully before completing any forms. Congratulations on being awarded scholarship funds to help defray costs forattending PHA’s 9th International PH Conference! Individual award amounts vary. Please refer to the notification letter enclosed in this packet to find out the total amount of your scholarship award. Scholarship funds will be applied first to your conference registration fee, second to hotel costs and third to travel costs, up to the total amount of your individual award. Any funds remaining in yourindividual scholarship award after paying these costs will remain in the PHA Scholarship Fund. Please review the following carefully, as it contains important information on how to utilize your scholarship award and make arrangements to attend Conference. In previous conferences we have noticed that partial awards are often not sufficient for some patients to attend. As soon as possible, please determine what your full cost of attending Conference will be (including registration, hotel and any travel expenses). If the amount of scholarship funds awarded will notenable you to attend please let PHA know immediately so funds can be distributed to another scholarship applicant. It is important
you confirm your plans with PHA as soon as possible, but no later than June 7, 2010. Be sure to consider all costs related to travel including, but not limited to: airline baggage fees, airport shuttles, in-flight oxygen costs, parking and if driving, gas and overnightlodging during the drive. How do I accept this scholarship award? To accept your scholarship award, please complete and return the forms included in this packet, indicating that you will attend the Conference and that you are able to cover the additional costs over and above your individual scholarship award. How do I register for Conference? Register for the Conference by completing andreturning the enclosed registration form to PHA, at the address above. Registration and hotel confirmations will be mailed to scholarship recipients in early spring. Any companions or family members that will be accompanying you, including children, must register and make their payment separately through General Conference Registration. Where is Conference being held? The Hyatt Regency Orange Countywill be hosting PHA’s 9th International PH Conference and Scientific Sessions. Scholarship recipients, who have been awarded sufficient funds to cover their hotel costs, should complete the enclosed formsto make their hotel reservations. PHA will make hotel reservations for you at the Residence Inn Anaheim based on the information you provide here. PHA funds will cover your hotel room costs for up to four nights (June 23 – 26, 2010).
