Ojo seco: diagnóstico y tratamiento
RESUMEN
Hemos revisado la bibliografía más actual sobre esta patología tan frecuente como es el ojo seco, así mismo damos una serie de consejos para el manejo de este tipo de enfermos. Lo más importante a reseñar es que es un problema muy frecuente (30% de las consultas deoftalmología) y que ante la sensación de picor, dificultad para abrir los ojos por las mañanas, sensación de arenilla y sobre todo si aparecen en personas de más de 40 años deberemos pensar en esta enfermedad. Su tratamiento es difícil ya que raramente es etiológico y las más de las veces nos tenemos que conformar con un tratamiento paliativo pero bastante eficaz, aunque se deberá mantener de porvida. Las lágrimas artificiales son la mejor y muchas veces la única alternativa para el tratamiento de estos enfermos. En el mercado farmacéutico existe una amplia gama de productos con la finalidad de dar mayor humedad al ojo, nuestro trabajo consistirá en aconsejar al paciente cual es el más apropiado para él. Desde pomadas oftálmicas nocturnas, lágrimas con o sin conservantes, lágrimas másespesas o más líquidas tendremos que ir buscando la que mejor tolerancia presente y de más alivio a nuestro paciente. También en ocasiones hay que tratar enfermedades que suelen acompañar y agravar a un ojo seco como es la blefaritis, alteraciones palpebrales (ectropión, entropión, distriquiasis), neurológicas (parálisis facial) e inmunológicas. ABSTRACT
We have checked the most current bibliography on this so frequent pathology as is the dry eye, also we give a series of advice for the managing of this type of sick. The most important to outline is that is a very frequent problem (30% of the ophthalmology consultings) and that in the face ofitching sensation difficulty to open the eyes in the morning, sand sensation and above all if appears in greater persons of 40 years, we will have to think about this disease. Its treatment is difficult sice rarely it is etiological and the more than the times we must resign ourselves with and quite effective but paliative treatment, though it should be maintained of by life. The artificial tears arethe better, and many times the only one, alternative for the treatment of these sick. In the pharmaceutical market exists a wide rade of products in order to give greater dampness to the eye. Our work will consist of advising to the patient which is the most appropiate for him. From nocturnal ophthalmic ointments, tears with or without preservatives, tears more thicken or more fluid, there willbe that to go proving the one which present better tolerance, and give more relief to our patient. Also, in occasions, one must to try diseases that tend to accompany and to aggravate to a dry eye, as is the blefaritis, eyelid alterations (ectropion, entropion, distriquiasis), neurological (facial, paralysis) and inmunological alterations. INTRODUCCION Elojo seco es la patología más frecuente que nos encontramos en la consulta de oftalmología. Se calcula que hasta un 30% de las personas que acuden a nuestra consulta lo hacen por este motivo, sobre todo personas mayores, si bien se manifiesta con gravedad en un porcentaje mucho menor. La sensación de picor, arenilla, sequedad ocular, enrojecimiento conjuntival, dificultad para abrir los ojos por lasmañanas son molestias que aunque no son específicas del ojo seco, son muy orientativas de esta patología. Ante este abanico de quejas lo primero que debe hacer el médico de atención primaria es pensar que lo que puede estar padeciendo esta persona es un ojo seco.
