Obesidad: trascendencia y repercusión médico-social

RESUMEN

La obesidad es uno de los principales problemas de salud pública en todo el mundo. En la actualidad se considera la pandemia del siglo XXI. Anteriormente se consideró un signo de belleza y salud, pero en la actualidad conlleva grandes problemasasociados. En Estados Unidos existen más de 97 millones de adultos obesos (54% de la población; 13% de los niños padece obesidad). En México, más de 70% de la población adulta tiene obesidad y sobrepeso. Su origen incluye factores genéticos (principalmente el gen de la leptina y de su receptor), y ambientales (estilo de vida: hábitos alimenticios y actividad física). Implica una gran comorbilidad,como diabetes, hipertensión y enfermedad cardiovascular, ejes importantes del síndrome metabólico. Para su diagnóstico deben consi- derarse los parámetros establecidos por la Norma Oficial Mexicana (NOM) para el tratamiento de la obesidad, con base en el IMC: entre 18 y 24.9 es normal, pero cuando es mayor a 25 y menor a 27 es sobrepeso, y mayor a 27 implica obesidad. Su tratamiento comprendecuatro estrategias importantes: modificación en los hábitos alimenticios, incremento de la actividad física, tratamiento del aspecto psicológico y detección y tratamiento de enfermedades subyacentes asociadas con la obesidad. En casos extremos se ha reportado tratamiento farmacológico y la cirugía bariátrica

The obesity is one of the most important healthy problems in the world. It is the biggest pandemic of the twenty first century. In the past, it was considered a sign of the beautiful and healthy life, currently the obesity is associated with big health problems. The sta- tistics in United States reported more than 97 millions of obese adults, the 54% of the total population, and the obesity ininfants is approximately of 13%. In Mexico, more than 70% of adults have obesity or overweight. And the infantile obesity has been increased to the first in the world. The etiology has included genetic factors. Many genes have been found associated with obesity such as the leptine gene and his receptor. Other factors associated are environment factors, food habits and physical activity. The obesityhas relationship with diabetes, hypertension, cardiovascular disease, risk factors of metabolic syndrome. To diagnose obesity it is necessary to calculate the body mass index (BMI) according to the Mexican Official Norm (NOM) to the management of the obesity, BMI from18 to 24.9 is considered normal. From 25 to 27 is considered overweight, and more than 27 is considered obesity. The obesitytreatment includes four essential bases: modification in the food habits, increase the physical activity, management the psychological aspects and detection and treatment of the diseases associated with the obesity. When the obesity is extreme it is necessary to use obesity drugs such as sibutramine or orlistat and the bariatric surgery.
