General concepts.

When you perform a suture must have enough material of synthesis; enough threaded needle and the choice of the same will be done according to the wound; if it is superficial or deep, cutting or ripped, with or without loss of substance.

The suture threads are prepared in various forms, as for example: in packages of 20 strands with a length of 40 cm labeled; in containerswith lids directly by placing the reel of thread embedded with an antiseptic solution or in vapors of formalin.

The sutures to separate points can be done in two ways: using a thread for each stitch (when tie with the hands), or with a single thread for many stitches (if you do it with the needle holders).

The size of the needle would be in relation to the scale of the "takes", the moretissue should be taken to include the stitch, larger must be the needle. In regard to the curvature, the needles strongly curved (circle) are used for sutures of depth and the little curves (1/3 circle) as well as the straights, are used for surface sutures. In regard to the tip, the triangular tip needles are used to stitch the tough fabrics and the round-nose for delicate tissues. They are also theatraumatic needles to minimize tissue trauma given by the passage of the needle eye-level. The suture material that follows the needle has the same section as this.

There are the tree types of sutures that we are going to talk about.

Cross Suture.

Continuous Suture.

Simple Suture.

Types of processing.

-These can be:
-Manual: with one or both hands.-Instrumented: Needle Holders, hooks, needles suture thread. In case of use nylon as suture thread, it requires a little more care because it is a material that is too rigid and could unrigged easily.

Purposes.

Stop a hemorrhage or close a wound.

Essential Steps to follow at the time of suture.

-Always shall be composed of a minimum of three knots at the time of preparation of a knot you mustapply sufficient force from the second knot to approximate the edges of the wound without compress too therefore the suture should include a thickness of tissue such that the movements of the patient does not become loose when you make an effort.
- After tying the knot last the leftover parts of the suture material should be cut to 2 or 3 mm. Allowing the knot is knotted in case of inclined thesuture material.
- The distances of the sutures in a general way must be: 1 for 1 for skin, previous aponeurosis, peritoneum; 3 by 3 in order to avoid muscle necrosis, and adipose tissue to prevent infection.

Post-operatory care, recommendations before and after the suture.

-Use antibacterial soap and cold water; gently clean your injury to keep free of germs.
- Keep the stitches coveredwith a bandage for 24 to 48 hours, unless your doctor authorizes you to do so.
- Keep the stitches and bandages clean and dry.
- Try not to hurt or touch constantly the stitches area; this may cause the tearing of the injury.
-Do not cut off the ends of the stitches.
- If the stitches are in contact with your clothes, place a gauze bandage between the sutures and your clothes.
- Ask your doctorif you need an antibiotic ointment or other type of medication.
- Limit activities that stretch the areas around your wound. This will help to prevent bleeding or swelling of the injured area.
- If your injury is located on your arm or leg, raise the injury above his heart. This will help decrease the pain and swelling. You can use a pillow to raise your arm or leg while sitting or having rest.- Reduce the scar: Do not touch your injury. Use sunscreen if you expose the lesion in the sun. Apply it every day after the sutures are removed. This will help to prevent discoloration of your scar.
