Aneurisma cerebral posterior izquierdo segmento P2

RESUMEN
Objetivo. 1. Conocer, mediantela presentación de un caso, las distintas opciones para abordar quirúrgicamente a esta patología. 2. Destacar las dificultades y los cuidados que se deben tener al momento de abordar este tipo de aneurismas.
Descripción. Paciente mujer de 48 años, que ingresa por cefalea, vómitos, rigidez de nuca, diplopía (Hunt y Hess II) y TAC de ingreso que muestra hemorragia subaracniodea (Fisher III). Conangiografía cerebral que evidencia aneurisma del segmento P2 de la arteria cerebral posterior.
Intervención. Es intervenida quirúrgicamente realizándose un abordaje subtemporal consiguiéndose el clipado del aneurisma sin comprometer el segmento distal de la arteria.
Conclusión Los aneurismas del segmento P2 son muy poco frecuentes y ofrecen grandes dificultades técnicas para su abordaje debido a suubicación en la profundidad del lóbulo temporal en relación con las cisternas peritroncales. Todas las medidas que faciliten una menor retracción cerebral constituyen junto con el minucioso conocimiento anatómico de la zona, la base para el abordaje de los mismos, obteniéndose los mejores resultados.

ABSTRACTObjective. In the resolution of the aneurismatic pathology the election an adequate surgical strategy is still important, especially on those with difficult access due to the anatomical location; this is the case of the aneurisms of the segment P2. The objectives of this work are: 1. To know the different options so as to treat this pathology surgically. 2. To highlight the difficulties and caresthat should be taken into account at the moment of treating this kind of aneurism.
Clinical presentation. A case of a 48 year-old female patient is presented. The patient was admitted with migraine, vomit, nape rigidity, and diplopia (Hunt & Hess II). Admission CT shows subaracnoid hemorrhage (Fisher III). Cerebral angiography evidences an aneurism of the segment P2 of the Posterior CerebralArtery.
Intervention The patient is surgically intervened through a subtemporal approach and g the aneurism is clipped without compromising the distal segment of the artery.
Conclusion. The aneurisms of the P2 segment are very infrequent and offer great technical difficulties to be treated due to their location in the deep parts of the temporal lobe in relation to the peritroncals cisterns. All themeasures that contribute to a lower cerebral retraction constitute, with the meticulous anatomic knowledge of this zone, the base for its treatment, obtaining the best results.

INTRODUCCIÓN
Los aneurismas de la arteria cerebral posterior son raros, su frecuencia se ubica entre el 0,7% al 2,3% de todos los aneurismasintracraneales1.
La arteria cerebral posterior desde un punto de vista neuroquirúrgico, puede dividirse en tres2 o cuatro segmentos3,4; esta última división es la más utilizada, compuesta por: el segmento P1 comprendido entre el origen de la arteria cerebral posterior y su unión con la arteria comunicante posterior (AcomP). El sector P2 comienza a nivel de la AcomP y finaliza cuando la arteria llegaa la cisterna cuadrigeminal. Este segmento se divide en dos porciones: anterior o P2a y posterior o P2p. El límite entre ambos es el borde posterior del pedúnculo cerebral y cada sector mide cerca de 25 mm. La porción P3 se inicia en la cisterna cuadrigeminal y se extiende hasta el inicio de la cisura calcarina. El segmento P4 corresponde a la distribución cortical de las ramas terminales.
