SINDROME DEL TUNEL CARPIANO

RESUMEN:
El síndrome del túnel carpiano es una dolencia provocada por la inflamación y la presión en el interior del túnel formado por el carpo y el ligamento carpiano transverso en la muñeca, donde se encuentran diversos tendones y el nervio mediano. Es una neuropatíaperiférica focal muy común, frecuente en mujeres (7:1) entre 40 y 60 años y relacionado con la ocupación. Afecta a más de 5 millones de norteamericanos. La compresión del nervio puede deberse a tenosinovitis, luxación articular
del semilunar, fracturas viciosamente consolidadas (Colles), esguinces o artritis, uso continuo de la
muñeca en flexión mucho tiempo. Otros factores causales pueden serArtritis Reumatoidea, diabetes,
stress laboral y el uso repetido de herramientas manuales de vibración entre otros. Clínicamente se
manifiesta con dolor, adormecimiento de los dedos, cosquilleo, debilidad y ardor en manos y dedos,
con mayor frecuencia durante la noche. La semiología es clave para su diagnóstico a través de los
signos de Phalen y Tinel-Hoffman. La Electromiografia es obligatoriaen caso de dudas. El tratamiento se inicia con la aplicación de férulas durante la noche, antiinflamatorios para aliviar el dolor y entumecimiento. También puede usarse una inyección de corticoide para reducir la inflamación. Es imprescindible que la muñeca permanezca en una posición neutra (articulación de la muñeca recta y no
hacia abajo). Si los síntomas son graves o no mejoran con eltratamiento mencionado, se recomienda intervención quirúrgica convencional o endoscópica. Otras alternativas disponibles son ultrasonido
y láser.

The syndrome of the carpiano tunnel is an ailment caused by the inflammation and the pressure in

The semiología is key forits diagnosis through the signs of Phalen and TinelHoffman. The electromiografía is obligatory in case of doubts. The treatment begins with the application of splints during the night, antiinflammatory to alleviate to the pain and entumecimiento. Also a
corticoid injection can be used to reduce the inflammation. It is essential that the wrist remains in a
neutral position (joint of the straightwrist and not downwards). If the symptoms are serious or they do
not improve with the mentioned treatment, conventional or endoscópica operation is recommended.
