Importancia del tratamiento rehabilitador multifactorial en la esclerosis lateral amiotrófica

THE IMPORTANCE OF MULTIFACTORIAL REHABILITATION TREATMENT
IN AMYOTROPHIC LATERAL SCLEROSIS
Summary. Introduction. The treatment of amyotrophic lateral sclerosis (ALS) is still a major challenge. Rehabilitation treatment is scarcelyconsidered and its usefulness in these patients continues to be controversial. Patients and methods. A multidisciplinary team made up of neurologists, physiotherapists, logopaedists, defectologists, psychologists and specialist physicians treated six patients with ALS in an intensive rehabilitation programme of 41 hours per week for four weeks. We follow certain basic principles in rehabilitationincluding: 1. Treatment by a multidisclipinary team; 2. Treatment tailored to the individual; 3. Avoidance of muscle fatigue and vigorous exercise, and 4. Intensive treatment with carefully measured amounts of different activities (logophoniatrics, occupational therapy, psychology, physical therapy, etc.) to avoid fatigue. The patients fulfilled the criteria of E1 Escorial for the diagnosis ofdefinite ALS, and gave their informed consent to undergo the treatment. Forced Vital Capacity (FVC) and ALS Functional Rating Scale (ALSFRS) tests were done on all patients at the beginning and end of the treatment. The Wilcoxon test for paired series, comparing scores at the start and finish, were done in each case. Results. In all patients the FVC and ALSFRS improved after the treatment. Theresults were statistically significant (Z: 2.2013; p= 0.027) on the Wilcoxon paired series test and no complications were seen. Conclusions. Intensive, multifactorial rehabilitation treatment for four weeks improved the FVC and ALSFRS in all patients with ALS and no complications were seen. INTRODUCCIÓN El tratamiento de la esclerosis lateral amiotrófica (ELA) continúa siendo un gran reto, a pesar de que las primeras descripciones de esta enfermedad las realizó Charles Bell hace más de 170 años [1]y de ensayarse numerosos tratamientos durante todos estos años [2]. La publicación de ensayos clínicos donde por primera vez en la historia de esta enfermedad se planteaba la posibilidad de retardar el curso de la misma, generó un gran optimismo dentro de la comunidad médica, a pesar de que sólo demostró un pequeño efecto positivo en la supervivencia [3]. Actualmente se llevan a cabo múltiplesinvestigaciones en las que se cifran grandes esperanzas [2]. Sin embargo, el tratamiento rehabilitador es apenas abordado y su utilidad con estos pacientes continúa motivando contro­ versia. Algunos ensayos plantean que el tratamiento rehabilita­ dor multifactorial permite mantener a los pacientes con un nivel funcional óptimo el mayor tiempo posible y ayuda a prevenir las complicaciones propias deldesuso de los músculos y la inmovilización [4,5]. También señalan que hasta que no se encuentre el tratamiento curativo de la ELA, la rehabilitación multifactorial

permanece como la mejor esperanza para mejorar la salud y la supervivencia en estos pacientes [6].
PACIENTES Y MÉTODOS En la clínica de enfermedades neuromusculares del CIREN (La Habana, Cuba), con un equipo multidisciplinariodirigido por un neurólogo y constituido por fisioterapeutas, logopedas, defectólogos –encargados de la terapia ocupacional en estos pacientes–, psicólogos, médicos internis­ tas y enfermeras, sometimos a 6 pacientes con ELA a un programa de tratamiento rehabilitador intensivo, durante 7 horas diarias, de lunes a viernes, y 3 horas los sábados, durante 4 semanas.
