SUMMARY
The healthcare quality can be measured based on thefill-in of medical record (HC), a medical-legal document
that registers the identification of the patient, its health, disease, diagnosis, evolution and therapy. For it
the medical audit of secondary sources is made as a study of quality of fill-in. Objective: To evaluate the
quality of data registered in HC of outpatient care of new patients attending consultation at General
Pediatrics at theHospital Nacional Cayetano Heredia (HNCH). Material and Methods: A descriptive
retrospective cross-sectional study was made, including 384 HC selected by simple random sampling with
replacement. We considered the following dimensions: general patient data, anamnesis, physical
examination, diagnosis, work plan, treatment and identification of the medical doctor; the variable was
measureddichotomously (presence or absence of the variable). Results: The reason for consultation, weight
and height of patient were present in all HC. Date, age, and physical examination were present in more of
90%, the name and number of HC in less of 50%, whereas the biological functions and vital functions in
less of 20%. The diagnosis was registered in 99.2% and the work plan in 84.6%. Pharmacologicaltreatment
was complete in 7.4%. The identification of the doctor was completed only in 49.7%. Conclusions: The
quality of the data registered in the HC of new patients of outpatient care General Pediatrics in HNCH is
deficient, being necessary to implement mechanisms that allow of effective way to the doctors and
multidisciplinary healthcare team to monitor and improve the quality of care whichthey offer to his

RESUMEN
La calidad asistencial se puede medir basada en el llenado de la historia clínica (HC), documento médico-legal que
registra la identificación del usuario, su salud, enfermedad, diagnóstico, evolución y terapia. Para ello se realiza la
auditoría de fuentessecundarias como un estudio de calidad de llenado. Objetivo: Evaluar la calidad de los datos
registrados en HC de atención ambulatoria de pacientes nuevos atendidos en consultorios externos de Pediatría
General en el Hospital Nacional Cayetano Heredia (HNCH). Materiales y métodos: Se realizó un estudio transversal
descriptivo de corte retrospectivo, incluyendo 384 HC mediante un muestreo aleatoriosimple con reemplazo. Se
consideró las siguientes dimensiones: datos del paciente, anamnesis, examen físico, diagnóstico, plan de trabajo,
tratamiento e identificación del médico tratante, realizando una medición dicotómica de la variable (presencia o

ausencia de la variable). Resultados: Se encontró que el motivo de consulta, el peso y talla del paciente estuvieron
presentes en todas las HC. La fecha, edad, y examen físico estuvieron presentes en más del 90%, el nombre y
númerode HC en menos del 50%, mientras que las funciones biológicas y funciones vitales en menos del 20%. El
diagnóstico fue registrado en 99,2% y el plan de trabajo en 84,6%. El tratamiento farmacológico se encontró
completo en 7,4%. La identificación del médico se encontraba completa sólo en 49,7%. Conclusiones: La calidad
