What happened?
The injured worker (IP) was assigned to install ZPE-460 jack for tendon stressing. His lower leg was slashed by the strand. Aminor wound and bleeding was caused and no other symptom was found after treatments. During the accident, IP was setting a hydraulic jack (435kg) with 2 co-workers. The stressing tendon was located inheight of 1.2m from the working platform in which the width of platform was 2m. When IP closed to the tendon to carry out the setting works, the spared strands of adjacent tendon slashed his rightlower leg.
Findings:1. 2. The worker (IP) was wared a shorts (not uniform) when the accident happened. The adjacent side (almost 800mm apart) of the stressing tendon have another tendon was readiedfor the following stressing procedures, i.e. anchor head was installed and the wrapping sheet of strands were removed.
Remedial Action, Preventive Action & Improvements:

Recommend to wear theCompany’s uniform in which the trouser shall be wear for site works. Wrappings of strands after threading shall not be removed before stressing. Especially for the distance between tendons are close.Working platform shall be kept neat and tidy at all the time.
