Long-term Effects of Traumatic Injuries to Incisors and Periodontal Tissues during Childhood
A Retrospective Study

Abstract Aim: The purpose of this study was toinvestigate the incidence of dentoalveolar growth disturbances, loss of teeth and esthetic impairment after the end of growth following traumatic injuries to incisors and periodontal tissues during childhood. Material and Methods: 41 patients having sustained dentoalveolar trauma before age 10 and who were now at least aged 16 years, and a total of 68 traumatized teeth were documented by clinicalexamination, dental casts and photographs. We determined the three-dimensional position of the traumatized teeth on the casts, as well as the traumatized teeth’s pulp sensibility, percussive sound and sensitivity, shape and color. Results: 82% of the traumatized teeth were in the upper dentition. 45% of the traumatized teeth experienced subluxation, nearly 30% luxation, 16% avulsion. At the time ofthe follow-up examination (mean 17.0 years post-trauma), 57% of the traumatized teeth were still in situ. Immediately after trauma 7.4% of the teeth were lost; up to 16 years post-trauma 35.6% of the teeth were lost because of failed root canal filling, root resorption or ankylosis. We observed no functional deviations. Dentoalveolar growth disturbances were rare. Three teeth were markedlydiscolored. Conclusion: Following dentoalveolar trauma during childhood, negative effects on growth, function and esthetics can be minimized by timely interdisciplinary treatment and by follow-up controls until the end of growth to achieve an outcome satisfying both the patient and clinician. Zusammenfassung Ziel: Es sollte geklärt werden, wie häufig nach einem kindlichen Trauma Hemmungen der Kieferentwicklung, Zahnverlust und ästhetische Beeinträchtigungen nach Abschluss des Wachstums festgestellt werden können. Material und Methodik: 41 Patienten, die vor dem zehnten Lebensjahr ein Frontzahntrauma mit Parodontbeteiligung erlitten hatten unddie nun mindestens 16 Jahre alt waren, mit insgesamt 68 traumageschädigten bleibenden Zähnen konnten klinisch und mit Modell- und Fotodokumentation nachuntersucht werden. Die dreidimensionale Zahnstellung auf dem Modell, Sensibilitätsprobe, Perkussionsschall und -empfindlichkeit, Form und Farbe der traumatisierten Zähne wurden bestimmt. Ergebnisse: 82% der Traumata betrafen die obere Dentition. 45%der traumatisierten Zähne erlitten eine Subluxation, fast 30% eine Luxation, 16% eine Avulsion. Zur Zeit der Nachkontrolle (MW 17,0 Jahre nach Traumaereignis) waren 57% der traumatisierten Zähne in situ. Direkt posttraumatisch gingen 7,4% der Zähne verloren, 35,6% bis zu 16 Jahre post Trauma aufgrund von fehlgeschlagenen Wurzelfüllungen, Wurzelresorption oder Ankylosen. Funktionelle Abweichungenwaren nicht, Wachstumshemmungen im Alveolarbereich kaum zu finden. Drei Zähne zeigten stärkere Farbveränderungen. Schlussfolgerung: Nach einem kindlichen Frontzahntrauma lassen sich bei zeitgerechter, dem Trauma entsprechender interdisziplinärer Behandlung und häufigen Kontrollen bis zum Wachstumsende negative Folgen für Wachstum, Funktion und Ästhetik so weit minimieren, dass ein für Patient undBehandler zufriedenstellendes Ergebnis erreicht werden kann.
