PLAN DE CUIDADOS ESTANDARIZADOS PARA EL PACIENTE
CON ENFERMEDAD PULMONAR OBSTRUCTIVA CRÓNICA

RESUMEN
Este plan de cuidados está especialmente ideado para ser aplicado a pacientes con Enfermedad Pulmonar Obstructiva Crónica, aunque pensamos puede ser utilizado en otros procesosrespiratorios. En él hemos tratado de identificar los problemas más prevalentes que presentan los pacientes aquejados de esta afección, expresados como diagnósticos de enfermería codificados, siguiendo la taxonomía NANDA, dispuestos según el modelo de necesidades de Virginia Henderson, adaptado a nuestra praxis enfermera en atención hospitalaria. A estos diagnósticos se llega mediante un contactoinicial con el paciente a través de la cumplimentación del Informe de Valoración de Enfermería al Ingreso instaurado en el centro y diseñado sobre la base del mismo modelo.
Los objetivos se plantean como objetivos del paciente, es decir, resultados a alcanzar por el paciente.
Para alcanzar estos objetivos, enfermería realiza una serie de intervenciones definidas en la Clasificaciónde Intervenciones de Enfermería y dentro de cada intervención elegida se han codificado las actividades. Así mismo, en cada actividad se indica con la frecuencia que debe realizarse y el responsable de su ejecución; por razones de extensión no es posible su publicación integra.
El plan de cuidados será necesario individualizarlo según las necesidades particulares de cada paciente y seinstaurará en las primeras 24 horas del ingreso del paciente, las revisiones al plan se realizaran cada 48 horas, manteniendo las intervenciones indicadas si persiste el problema o cesándolas si se ha resuelto el mismo, según evolucione el proceso hasta el alta.
This Care Plan is especially designed to be applied to patients of “Chronic Lung Obstruction Illness”, although we think that itcan be used for other respiratory diseases. With this plan, we have tried to identify the most common problems present in all patients suffering from this disease, expressed as codified nursing diagnosis, following Nanda Taxonomy and displayed according to Virginia Henderson’s model of needs and adapted to our nursing praxis in hospital care. These diagnosis are reached through an initial contactwith the patient after filling out the nursing Assessment of Entry Form which has been established in the centre and designed after the same model.
Objectives are set as patient’s objectives, i.e., the results to be reached by the patient.
In order to reach these objectives, the infirmary makes a series of interventions defined in the classification of infirmary interventions and for eachchosen intervention the activities have been codified.
Likewise, the frequency in which the intervention must be made and the responsible for its administration are also indicated in each activity. However, due to its long extension the full-length version cannot be published.
The Care Plan will need to be individualized according to the particular needs of each patient and will beestablished within the first 24 after the patient has gone into hospital. The plan will be reviewed every 48 hors maintaining the indicated interventions if the problem persists or stopping them if the result is the same according of the evolution of the process until the departure of the patient.
